Medical Form Instructions
Pack 382 is required to have all Pack activity and event participants complete the Scouting America Annual Health & Medical Record - Part A & B. We will keep them in a secure file and ensure the forms are updated at least annually.
In order to make this easier for families each year when it needs to be updated, please follow these instructions:
Please make sure you have a copy of Adobe Reader so you can properly fill out the forms. You can find that application here: https://get.adobe.com/reader/ This link will work for both Windows and Mac.
Download and save a copy of the fill-able PDF located HERE (Part A & B)
*For Webelos Resident Camp - IF YOU ARE CAMPING over 72 hours, you will need the Part A, B & C found HERE (Part A B & C)Open the fill-able PDF, fill out the details on the form, be sure to save it, then print it and sign. It is ready to turn in.
The Pack needs a form filled out for every participant at Pack events. You'll at least need to provide a completed form for your Scout and their Akela (accompanying parent/guardian). To fill out a second (or more) form: (1) Open the completed form, the go to the File menu and choose Save As, and change the file name to match the name of the new person, then save it. (2) Now that the file name is updated, update the personal and medical details on the form, save again, and then print and sign to turn in. (3) Repeat for every member of your household.
Here’s the best part! Next year, when you have to update the form again, just do this:
Open the latest version
Save As and change the date
Update the details and save, print, and sign. No need to fill it all out again!
Submit forms by bringing them to a Pack Meeting or drop off to the Committee Chair’s house.
